Before your visit to Right Care, you will need to make sure you have the following information:
- Government issued ID (driver’s license, passport, etc)
- Insurance Card (if applicable)
If the patient is a child, the parent’s or guardian’s ID will be required. If someone other than the parent or guardian is bringing the child, we will need to speak with the parent or guardian to obtain permission to treat.
Additionally, please review the Patient Demographics, Past Medical History, and HIPAA forms below. You may print this out and complete them to speed up your registration process. Please remember that the information you provide regarding your past medical history is very important to our ability to provide you will the Right Care. Make sure to include any hospitalizations, surgeries, allergies to medicines, current medicines with dose and how often you take them, and any family history. Please do not list your family history as “deceased.” Please list what illness resulted in the death of your loved one.
